IVF 2: Immune stuff

Today I went for my pre transfer intralipids infusion. It’s part of the borderline protocol from Dr S, for patients who have a borderline reaction to pregnancy. When they did the immune testing it was found that I had an elevated level of NK cells in a pregnancy situation, so whilst I’ve been doing IVF, I’ve also been doing some immune treatment on the side.

Up till today it was just taking some extra supplements. Then because it looks like I’ll have my egg collection next week, which means a potential transfer end of next week, I’m meant to get the intralipids 7-10 days before. Now who knows if the timings are correct – I might not end up having the egg collection until end of next week, but I was getting stressed about timings because I can’t have it tomorrow (they’re not doing it as the midwife isn’t in) and she thought I should do it on Monday or Tuesday otherwise, and I’ll only find out tomorrow if the transfer’s on Monday or Tuesday as they think it will be. So… It seemed easier, especially as they upped my stims dose to 300 last night, to think of it happening on Monday or Tuesday. Intralipids it was.

I finished up at work and headed off to the clinic (private clinic outside of London) for the afternoon. The good thing about working my notice is that I’m way more flexible in what I can do! At the moment I’m working for a guy I’m friendly with, and I’m doing him a favour, and they aren’t going to put me on any projects during my notice period, so it’s easy enough.

One thing I’ve noticed with this stims phase is that I’m so tired. I mean, dog tired. (Though my dog spends all day sleeping so not sure he’s ever got to the tired stage.) I went and got the infusion and it’s given via IV. I was so tired that I just sat back in the chair and snoozed for about an hour!

The IV intralipids…

White stuff infusing into me!

I really think I may have snored a bit. I was sooooo tired! Also, my veins are a bit small and the infusion takes a while if you’re me. (I actually used to be a blood donor until they were like “Thanks but no thanks!” as it took soooo long to get half a pint!)

In terms of the sensation, it did start to ache a bit. I mean, it was going on for around an hour. But not so much that I didn’t just doze off! So on balance it wasn’t that bad.

Also tonight I had my normal Menopur stims injection, but as of yesterday evening (after my first progress scan) they told me to increase it to 300 instead of the previous 250. So I guess they’re stimulating me a bit more!

The other thing (in addition to all the supplements which I take in the evening as part of the borderline protocol) was I starting the heparin injection. I did that this evening and it hurt! It’s a thicker needle (pre-loaded so no choice on needle) so I didn’t enjoy that much. I’m supposed to do those every day until the evening before egg collection, and then start again. Also if I have a positive pregnancy test (here’s hoping) I would have another intralipids infusion at 4 weeks.

In myself I’m feeling overall really tired. My stomach has swollen up to the point where I now look about 3 months pregnant, which is annoying! But I suppose it will go down after egg collection. My weight has gone up alarmingly… Again I suppose it’s the effect of the meds. I feel absolutely exhausted and can barely keep my eyes open. I’m telling myself it will pass! I have no idea how I managed to go through all this last year when I was working really long hours. Although I’m trying to remember and I think that maybe I felt different on Gonal F than I do on Menopur.

So that’s the update – I really hope this is going to end well! At the moment we are thinking egg collection is early next week. If the follicles keep growing at this rate we will hopefully have a few eggs!

Well they can’t have you going through all this and looking glamorous at the same time 😂 the bloating & tiredness sucks, but hopefully this time it will be 100% worth it.
Is this the first time you’ve done the intralipid infusion? I actually felt pretty good on Gonal-F so maybe the menopur is a bit more hardcore in the fatigue department. Go easy on yourself and rest up.

Yeah the intralipids are part of the borderline immune protocol. Basically I was just over the acceptable range of NK cells when they simulated pregnancy. (Don’t ask me how they do that.)

I was doing really long hours last time ad so maybe I just attributed the tiredness to that! Also I’m off coffee for the time being – I usually only have one double espresso in the morning as I can’t sleep if I drink it in the afternoon. So maybe it’s that!

I’m taking it a bit easier although probably getting up at the same time as I did last year – I was working away from home getting up early and working, whereas this year I’m getting up early and walking the dog!

Hopefully this all goes to plan, you get your retrieval and transfer within the 10 day mark! AND that you’ll be getting another infusion in a little over a month!!!
I had to laugh at your swollen as if you’re 3 months pregnant remark. I’m a week into my 3rd month and I barely have a little bulge!! Hope your swelling goes away quickly though. Good luck!!

Yeah the different meds are probably screwing with you differently. Plus when you have a lot on you tend to just push through and do your best to ignore anything pushing back! Fingers crossed that egg collection comes at the perfect time 🙂 Not long now!!! X

Thanks! Yes I’m hoping I have egg collection Monday or Tuesday. I have to say it’s a lot easier this time not working on a project. I hope that this is the last time I have to do it! X

Fingers crossed for lots of nice healthy eggs and a smooth collection!! I’ve never heard of intrilipids infusions – interesting! I was looking back at photos of me durig and after my stimulation, and I look soooo pregnant!! Mustve taken awhile to go away too coz all my aussie photos I look like Im about to deliver. definitely prompted an after shower mirror inspection. Nope not far anymore look haha. Even though it will all change anyway again 😂 all the best xx

Haha! Yeah I don’t even want to take a photo of me as I’m so bloated! Wearing all my “fat” dresses at the moment and don’t even want to think about putting on jeans! I’m hoping it goes down for a bit after the egg collection. Though obviously hoping it’s a proper pregnancy bump after that! X

  9. I really hope very best to u!! I am on immune protocol (ivig, not intralipid) and lovenox (low dose heparin). It looks like working for me after 4 losses on 6-7 weeks. I am already close to 9 weeks and feels like something is working! Hope you will get a wonderful result with this protocol😊❤️

Wow congratulations, so happy for you! Yes I think mine is low dose heparin. Not sure what IVIG is? I think it depends on what the tests show – mine was considered an only slightly abnormal reaction. But I’m hoping that it was the reason and is treatable. Wishing you all the best with your pregnancy!

  10. One more thing to add, drink a lot before, on, and after infusion day ( i drink 80 oz per day) to stay hydrated. It can get you headache, but drinking water and eatjng fruits can help you a lot!!

Mmmm intralipids. We give these to the NICU babies I work with and its so interesting to me that it’s a treatment for immune issues. You are in my thoughts and I wish you the best of luck with this upcoming retrieval and transfer!

Poor us hey! It’s knackering! Yeah I’m sleeping a lot. It’s strange as I’m someone who’s usually okay without too much sleep – I just catch up at the weekends. Hope we both do great!

  13. I’m pretty sure all that tiredness is because you have been growing lots of lovely follicles for the eggies to mature in!!!
Wow your borderline protocol is intense! But it will be worth every stab and pinch 😊 hang in there with the bloating!! I hope it doesn’t get too much worse for you🙃

Thank you! I hope that too! I assume it is happening otherwise I wouldn’t feel soooo tired! I actually feel slightly less tired today (so far – on way to my morning scan). It will definitely be worth it if we get a good result!
